What they cost you beyond the lower wage: limited choice of city and hospital, breakage fees of USD 20,000 or more if you leave early, and contracts that sometimes lock you into night shifts for the first year.

Direct-hire route: higher ceiling, harder entry

A direct-hire offer from a hospital like Houston Methodist, Cleveland Clinic, or HCA pays you the full posted rate from day one. You keep every dollar. The catch is that very few US hospitals will sponsor an EB-3 for a nurse who has never set foot in the country, has no US references, and still has CGFNS pending. Direct-hire usually works for Tanzania nurses who already hold a US RN licence by endorsement, who have a relative on a green card willing to anchor the application, or who first land on a J-1 or F-1 and convert.

Realistic plan if you want direct-hire eventually: use an agency for your first 24 months, finish the contract clean, then jump to a direct-hire role in year three when you have US experience, US references, and your I-140 already approved.
